Job Description
**Job Title** Community Rehabilitation Assistant **Role Summary** Supports adult patients within their homes, community hospital wards, and care homes to attain independence and improve quality of life. Works closely with a multidisciplinary team of nurses, physiotherapists, and occupational therapists to deliver and monitor intermediate care and rehabilitation interventions. Primary responsibilities include driving to patient sites, assisting with therapy goals, administrative support, and maintaining accurate records. **Expectations** * Deliver high‑standard, evidence‑based care under supervision, with increasing autonomous decision making. * Participate in continuous learning and statutory training. * Maintain confidentiality, professional conduct, and robust communication with patients and team members. **Key Responsibilities** 1. Implement planned interim care/rehabilitation for patients in home, community hospital, or other community settings. 2. Travel by vehicle to patient locations on a 22.5‑hour weekly rota, including weekends. 3. Collaborate with nurses and therapists to set and review patient goals, providing practical assistance to achieve them. 4. Manage own workload: maintain diary, record interventions, and document progress. 5. Assist clinicians with administrative tasks to support efficient team operations. 6. Record personal learning, identify improvement areas, and engage in reflective practice. 7. Attend mandatory and statutory training sessions as dictated by appraisal and trust policy. 8. Ensure adherence to Enhanced DBS requirements and ongoing professional standards. **Required Skills** * Valid UK driving licence and reliable vehicle access. * Experience in community rehabilitation or similar patient care role. * Strong organisational and time‑management capabilities. * Effective communication, empathy, and teamwork. * Ability to work independently while seeking feedback. * Basic IT literacy for record‑keeping and communication. **Required Education & Certifications** * Enhanced DBS check (private or submitted). * Relevant statutory training (e.g., health & safety, safeguarding). * Prior qualification or experience in health and social care (e.g., health care assistant, therapy assistant, nursing/physiotherapy/occupational therapy apprenticeship) is advantageous but not mandatory.
