**Job Title**
Health and Wellbeing Coach
**Role Summary**
Provide individualized health coaching to empower clients to take control of lifestyle behaviors impacting physical and mental wellbeing. Deliver face‑to‑face, telephonic, and digital support in one‑to‑one and group settings, focusing on nutrition, smoking cessation, physical activity, weight management, alcohol use, and NHS health checks. Use evidence‑based techniques such as motivational interviewing, goal setting, and problem solving to promote sustained behavior change and improve quality of life.
**Expectations**
- Deliver coaching that meets national standards, policy, and role boundaries.
- Manage a caseload of participants, ensuring appropriate intensity and frequency of contact.
- Maintain accurate, confidential records in the case‑management system.
- Engage in continuous professional development and meet agreed KPIs.
- Escalate or refer clients when coaching is unsuitable, ensuring seamless support continuity.
**Key Responsibilities**
- Conduct triage, assessment, and agenda setting.
- Facilitate lifestyle behavior change through motivational interviewing, goal setting, and problem solving.
- Deliver personalized coaching via face‑to‑face, phone, and digital channels, including in‑app, text, and email.
- Share resources, signpost services, and advocate use of behavior‑tracking tools.
- Adjust support levels based on daily caseload review and participant progress.
- Participate in community outreach, screening programs (e.g., NHS health checks), and stakeholder events.
- Implement SOPs, comply with data protection, health & safety, and safeguarding policies.
- Audit data and outcomes, contributing to service improvement and KPI attainment.
**Required Skills**
- Strong communication and motivational interviewing skills.
- Ability to assess needs, set achievable goals, and problem solve.
- Proficiency with digital platforms, multi‑channel communication, and behavior‑tracking tools.
- Accurate record‑keeping and data entry.
- Knowledge of NHS health checks and public health guidelines.
- Cultural competence and inclusive coaching approach.
- Team collaboration and ability to work independently.
**Required Education & Certifications**
- Degree or equivalent in Health, Allied Health, Psychology, Social Care, or related field, OR recognized health coaching certification.
- Valid health‑coaching or counseling qualification preferred (e.g., Certified Professional in Coaching, NVQ/HNC/HND in Health).
- Ongoing professional development and up‑to‑date knowledge of national health guidelines.
