Job Description
**Job Title:** Community Health Worker **Role Summary:** Engage health plan members in their homes and communities to improve utilization of health plan benefits, provide education, coordinate care appointments, and address barriers to care. Serve as a trusted field-based liaison between members and the health system, documenting interactions and ensuring accurate coding. **Expectations:** - Achieve high member engagement and satisfaction. - Facilitate timely scheduling of care visits and clinical appointments. - Resolve obstacles to benefit utilization and advocate for member needs. - Maintain accurate, confidential documentation in the internal EMR. - Operate independently with discretion, ensuring privacy and data security. **Key Responsibilities:** - Visit members at residences, senior living, or assisted living facilities to provide education on benefit plans and programs. - Encourage and schedule annual Optum at Home APC visits and other clinical appointments. - Build trust and strengthen relationships with Optum health care providers. - Identify and remove barriers preventing members from using benefits. - Proactively address potential challenges, escalating concerns when needed. - Complete Health Risk Assessments, Social Determinants of Health assessments, and identify additional health needs. - Perform Bone Density Screening – Osteoporosis Management in Women who had a fracture (OMW) when appropriate. - Document all engagement attempts and code accurately in the EMR. - Plan efficient routes to optimize time management and travel. - Use scheduling tools to secure member appointments, contributing to Stars gap closure and reduced medical expenses. **Required Skills:** - Strong interpersonal and communication skills with diverse populations. - Sound judgment and the ability to consult leadership and escalations. - Intermediate computer proficiency, navigating a Windows environment. - Vehicle ownership, valid driver’s license, and reliable transportation. - Ability to plan routes and manage time flexibly. - Discretion with confidential information and secure workspace. - Proficient in scheduling and route optimization tools. **Required Education & Certifications:** - High School Diploma or GED (or higher). - Minimum 3 years of community health or outreach experience. - Optional: Certified Community Health Worker (CHW) certification. - Preferred: LPN, CNA, Medical Assistant, or related health certification; bilingual fluency in Spanish/English. - Preferred: Experience with vulnerable populations, Medicare/Medicaid, and managed care environments.

Collaborates in a Scrum team, designs, implements, tests, and releases secure, scalable RESTful APIs and services using Java and Spring Boot 3.x on GCP. Expectations: Deliver committed features on time with high quality, maintain ≥80% test coverage, participate fully in sprint ceremonies, and mentor junior team members.
