Expectations: - 7+ years experience in eldercare, gerontology, benefits planning, or family care coordination. - Ability to set personal rates and manage self‑scheduling. - Demonstrated proficiency in trauma‑informed, family systems–based practice and meticulous documentation. Key Responsibilities: - Assess client risks and needs; produce comprehensive 30‑day care plans and provider maps. - Explain dementia progression, safety measures, behavioral strategies, and respite care options. - Compare and advise on senior living alternatives, budget implications, and move timelines. - Clarify Medicare and Medicaid coverage, savings programs, and appeal procedures. - Facilitate family role delineation, communication cadences, and documentation. - Collaborate with navigators to integrate care plans into individual client support plans.
