Job Description
**Job Title:** Registered Practical Nurse (RPN) **Role Summary:** Provide high‑quality, client‑centered in‑home nursing care to individuals transitioning from hospital to home in the Ottawa East area. Deliver personalized support for personal hygiene, wound management, bladder/bowel care, and post‑surgical recovery while collaborating with multidisciplinary teams. **Expactations:** - Maintain reliability and punctuality with flexible scheduling. - Demonstrate compassionate, patient‑focused care. - Communicate effectively with clients, families, and team members. - Adhere to all regulatory, safety, and documentation standards. - Keep professional liability insurance current. **Key Responsibilities:** - Assess and perform nursing interventions for personal hygiene, wound care, and continence management. - Monitor and document client health status, medication administration, and recovery progress. - Educate clients and caregivers on care plans, self‑management, and preventive measures. - Coordinate with physicians, therapists, and other health professionals to ensure comprehensive care. - Support clients in community activities and daily living as needed. - Ensure compliance with infection control, BLS/CPR protocols, and CNO regulations. **Required Skills:** - Strong clinical assessment and nursing care abilities. - Excellent interpersonal and communication skills. - Ability to work independently and as part of a multidisciplinary team. - Proficiency with smart‑phone technology for documentation and communication. - Time‑management and organizational competence. - Valid driver’s license (G class) and reliable vehicle (preferred). **Required Education & Certifications:** - Registered Practical Nurse (RPN) license, registered with the College of Nurses of Ontario (CNO). - Valid Basic Life Support (BLS) / CPR Level C certification. - Professional Liability Insurance coverage. - High school diploma or equivalent; nursing diploma/degree required.
