**Job Title**
Disability Benefits Navigator – GAIN Program
**Role Summary**
Provide individualized support to people with vision loss and their families through the application, tracking, and appeals of federal and provincial disability benefits (e.g., Disability Tax Credit, Canada Disability Benefit). Act as the main point of contact, ensuring timely access to services, healthcare connections, and community supports while maintaining accurate client records.
**Expectations**
- Deliver client‑centered, culturally respectful guidance on benefit eligibility and application processes.
- Maintain up‑to‑date knowledge of benefit programs and share updates with the team and partners.
- Participate in quality improvement, client satisfaction surveys, and multi‑disciplinary team meetings.
- Demonstrate organizational and advocacy skills, working independently or collaboratively.
**Key Responsibilities**
- Assist clients and families in understanding eligibility, completing forms, and coordinating required documentation.
- Monitor application status, conduct follow‑ups, and manage appeals where necessary.
- Document all service‑related activities in the client health record in accordance with organizational policies.
- Proactively engage with clients to ensure ongoing collaboration with healthcare and community service providers.
- Build and maintain professional relationships with external partners and referral sources.
- Offer training and resources to promote client independence and well‑being.
**Required Skills**
- In‑depth knowledge of disability benefit programs (e.g., DTC, CDB).
- Exceptional verbal and written communication; ability to explain complex processes clearly.
- Proficiency with case‑management systems, MS Office, and electronic health records.
- Strong organizational, problem‑solving, and advocacy abilities.
- Clinical understanding of client‑centered philosophy and intersectional equity principles.
- Ability to prioritize tasks, work independently, and collaborate effectively with multidisciplinary teams.
**Required Education & Certifications**
- Post‑secondary diploma in Social Services, Disability Studies, Community Work, or a related field.
- Minimum 2 years’ experience in client navigation, healthcare or social service systems.
- Experience supporting individuals with disabilities, especially vision loss.
- Experience working with diverse populations (Indigenous, 2SLGBTQIA+, newcomers, etc.).
- Preferred: Certification in Patient Navigation, trauma‑informed care, motivational interviewing, or health equity.
