Proud to be a member of the Alliance for Healthier Communities, we are a community governed non-profit resource centre providing health services, social supports and community development in an inter-disciplinary setting. We serve the constituency of South-East Ottawa, and particularly try to meet the needs of folks with limited access to health and social supports, including newcomers to Canada. Most of our services are free and many of our programs are operated both at our main location (1355 Bank St.) and out in the community. Our second site is EarlyON which serves families in the Heatherington-Fairlea community and our third site is at the Hunt Club-Riverside Community Centre. Job Description
**Job title:** Bilingual Care Coordinator **Role Summary:** Coordinate comprehensive, client‑centered care for individuals with complex needs in a community health setting. Utilize intensive case‑management techniques to align multidisciplinary teams, families, and clients around shared goals, ensuring seamless transitions across the health system. **Expectations:** - 14‑month contractual position covering maternity leave. - Fluency in English and French (additional languages a plus). - Current registration with an approved regulatory body (e.g., ONA, OCSW). - Proven ability to manage a caseload, adapt plans in real‑time, and act as a change agent. **Key Responsibilities:** - Manage a caseload of clients with complex needs via an intensive case‑management approach. - Facilitate collaboration among clients, families/supports, and multidisciplinary care teams to develop client‑centered care plans. - Conduct regular touchpoints to promote health literacy, support self‑management, and assess progress. - Adjust care plans in real time based on client needs and changes in circumstances. - Improve system transitions by strengthening communication and coordination among care team members. - Advocate for clients, providing information to support informed decision‑making. - Mentor and coach dispersed partner front‑line staff in adopting new tools and methodologies. - Implement change‑management activities to standardize processes and maintain quality. **Required Skills:** - Intensive case‑management and care coordination in a community/primary care setting. - Strong interpersonal and communication skills in English and French. - Ability to conduct comprehensive health or symptom‑directed assessments and synthesize data. - Experience with clients experiencing mental health and addiction challenges. - Problem‑solving, adaptability, and change‑management expertise. - Knowledge of social determinants of health and their impact on client outcomes. - Training in Non‑Violent Crisis Intervention, First Aid & CPR, Motivational Interviewing, ASSIST, or equivalent. **Required Education & Certifications:** - Post‑secondary education in health and/or social services. - Current professional registration with a recognized College or regulatory body (e.g., ONA, OCSW). - Diploma or continuing education in case management and/or care coordination (preferred). ---
