START are recruiting for two Community Connector roles for the VCSE Led Wellbeing Matters Social Prescribing Programme. We are seeking a Community Connector to work within the Walkden and Little Hulton community receiving referrals from health care professionals in the neighbourhood and a second Community Connector to work within Greater Manchester Mental Health (GMMH) ata specialist adult mental health inpatient facility at Salford Royal Hospital.

Both roles will initially be a fixed term contract, for 12 and 18 months respectively, with a view to securing longer term funding.

1x Walkden And Little Hulton Community

Full Time (35 hours), 12 month FTC £30,328 per annum (NJC Scale 19 prorated)

1x Salford Royal Hospital

Full Time (35 hours), 18 month FTC £30,328 per annum (NJC Scale 19 prorated)

About The Service

The Wellbeing Matters programme is a Voluntary Community and Social Enterprise led initiative, delivered by Salford CVS in partnership with Salford Third Sector Consortium, that focusses on Person and Community centred approaches to improving the wellbeing of Salford people.

The Wellbeing Matters Community Connectors are at the heart of our approach to social prescribing in Salford and we are looking for enthusiastic, organised, resilient and person-centred individuals to join our Wellbeing Matters team.

Working as part of the wider Wellbeing Matters team across Salford, the ideal candidate will be a caring, enthusiastic and connected individual with experience of using strengths or assets-based approaches in order to help service users to improve their health and wellbeing and (re)connect to their community.

Key Aspects of the Role:-

To work closely with individuals to understand their needs, goals and personal circumstances.
To recognise, respond to and appropriately report any safeguarding concerns in line with organisational policies and statutory requirements, ensuring the safety and wellbeing of all clients
To support residents by helping them access local groups, activities and services that improve health, wellbeing and independence.
To develop and maintain effective working relationships with relevant service providers to ensure effective and smooth onward referrals e.g. to other local voluntary, community or social enterprise organisations
To identify local gaps in meeting a specific need and feed this information into delivery team meetings
