Job Description
Job title: Support Worker Role Summary: Provide person‑centred support to adults with learning disabilities, autism, mental‑health or health conditions across multiple care settings. Deliver practical care, promote independence, facilitate community engagement, and assist in achieving individual goals while maintaining high standards of safety and dignity. Expectations: - Work flexible, varied shift patterns (day, late, sleep‑in, night, alternate weekends). - Participate fully in team meetings, care planning and reviews. - Demonstrate professional, reliable conduct and strong safeguarding awareness. - Pass requisite DBS, reference checks and pre‑employment verification before deployment. Key Responsibilities: - Assist with personal care (bathing, dressing, toileting) and daily living tasks. - Manage health needs and support medication or medical routines. - Promote independence, choice, and dignity in all interactions. - Facilitate access to local community services and support goal attainment. - Follow documented care plans, risk assessments, and clinical guidelines. - Collaborate with a multidisciplinary team to deliver high‑quality support. Required Skills: - Compassionate, reliable, and professional attitude. - Strong communication, interpersonal and teamwork abilities. - Flexibility in working varied shifts. - Knowledge of safeguarding principles and mandatory reporting. - Ability to assess and respond to individual needs and risks. - Commitment to maintaining confidentiality and data protection standards. Required Education & Certifications: - Completion of basic care training (e.g., NVQ Level 2 in Health & Social Care or equivalent). - Background check (DBS) and satisfactory references. - Additional safeguarding or working‑with‑disabilities qualifications are advantageous but not mandatory.
