About The Role

You will provide person-centred support to individuals with learning disabilities, autism, mental health needs, or health conditions across different care settings.

Shift Patterns

This Role May Include a Mix Of

Day shifts

Late shifts

Sleep-in shifts

Waking night shifts

Every other weekend workingFlexibility is essential to meet the needs of the service.

Key Responsibilities

Delivering high-quality support

Assisting with personal care, daily living tasks, and health needs

Promoting independence, choice, and dignity

Supporting individuals to access the community and achieve personal goals

Following care plans, risk assessments, and clinical guidance
