**Job Title:** Caseload Holder – Orthopaedic Discharge Support **Role Summary:** 30‑hour per week, temporary Band 6 registered nurse covering maternity leave for 12 months. Responsible for coordinating, organising and facilitating the safe and timely discharge of orthopaedic patients (elective and trauma) from inpatient care to community settings, working collaboratively with hospital, district nursing and multidisciplinary teams. **Expectations:** - Deliver patient‐centred care from admission to early discharge. - Ensure compliance with clinical, safety and documentation standards. - Maintain timely travel between sites (hospital, patient homes, clinics, surgeries). - Communicate effectively with patients, families and professional colleagues. **Key Responsibilities:** 1.
