Job Summary:
The Digital Transformation Manager will lead digital initiatives to modernise primary care services across Hillingdon's Primary Care Networks (PCNs) and GP practices, promoting integrated care delivery. The role emphasises enhancing patient access, optimising workflows, and fostering collaboration across the health and care system to improve efficiency, patient outcomes, and population health management in alignment with NHS England's digital priorities and North West London ICS objectives.

Primary Responsibilities
Lead the planning, deployment, rollout, and optimisation of digital transformation projects in Hillingdon primary care, with a focus on seamless integration and user adoption.
Support the deployment and ongoing PCN-level implementation of Blinx PACO (Patient and Care Optimiser), a cloud-based platform for online consultations, triage, patient communications, appointment management, and analytics.
Provide support to PCNs and practices in using EMIS Web, including system configuration, troubleshooting, and promoting adoption to reduce administrative burdens and enhance patient access.
Work across stakeholders to enable cross-working and integrated care, including collaboration with Central and North West London NHS Foundation Trust (CNWL) for community and mental health services, and The Hillingdon Hospitals NHS Foundation Trust for acute care pathways and being the digital lead into the ICB and national teams as and when needed
Support NHS App integration and rollout with Blinx PACO, ensuring patients can seamlessly link to online consultations and digital services via the NHS App as the primary digital front door.
Drive change management, stakeholder engagement (including clinicians, practice managers, PCN leads, and partner organisations), benefits realisation, and mitigation of adoption barriers.
Monitor project deliverables, risks, and KPIs, such as improved patient engagement, reduced demand on traditional channels, and better proactive care.
Identify opportunities for digital innovation, including automation, total triage, neighbourhood team integration, and interoperability with existing systems.
Support the research team to undertake complex searches as and when required and support in the digital development within the research function
Ensure adherence to NHS data security, information governance (including GDPR), cyber security, and accessibility standards.
Contribute to resource planning, procurement, and reporting on digital initiatives.
